Incident Reporting

How to report:

  • You can complete an incident report by following this link - Incident Report.
    or
  • You can send a message in #sensitive-discussions with details about the incident: who was involved, what happened, any other relevant information/observations.
    or
  • You can send a direct message or email to any of the board members with this information, board member names & emails are listed in the Incident Report linked above.

Incidents that should be reported:

  • Physical injury to patrons or volunteers
  • Situations where volunteers or patrons are made to feel emotionally uncomfortable
    • Ex: Patron makes inappropriate comments and is asked to leave for the day.
  • If the circuit breaker is tripped

You never should feel alone on a shift, you can always reach out to your co-volunteers for advice, or defer to people higher up the chain.

You can always ask someone to leave the facilities for the day (if a permanent ban is in order, we will usually designate someone to reach out to that person directly).

  • If they ask questions about that I will say something along the lines of: “I’ll have to ask the rest of the team about that. If you come back another day we might have an answer for you.”
